This is a quote from Kojima in the IGN interview: 'We asked him if the word "Porter"on Sam’s uniform is his last name, or his job.' “There’s some truth to it, and there’s some part that isn’t necessarily true to it,” he responded in the exact way we expected.'
OK, so the definition of porter is: a person hired to carry burdens or baggage, as at a railroad station or a hotel.
A hospital porter is: A hospital porter moves patients, equipment and numerous other medical paraphernalia between the various areas of a hospital. Hospital porters are essential in maintaining the smooth running of a hospital. A significant aspect of the job is patient care.
Sam (Norman Reedus) is a porter, by trade, and that is why the name is on his uniform. BUT, he is also a porter by design. Kojima has said there is something unique about Sam. “Most people in the game are aware of the rain -- and well, Norman is quite unique in this regard… I think I should stop there. I’m spilling the beans.”
I think Sam has the ability to travel between the different dimension or time frames or phases of life/afterlife or whatever it is in this game, and the ability to not be affected by the timefall like everyone else is. And the porter aspect of his character is perfect for transporting babies (whatever they may mean) to and from locations to different locations (dimensions, time frames, worlds, etc.).
