someone else may have already thought of this but I haven't heard it actually said yet so I'll just post it here, make what you will of it and remember this is pure speculation:
I wonder if The basic premise may be; we play as Norman Reedus who is from our real world and an anomaly of some type somehow pulls him into the reality of death stranding (real life Norman Reedus could break the fourth wall with this one by "disappearing" when the game is released though I think that would be a bit extreme) in the world of death stranding death has little meaning as people aren't actually able to die they just enter a reverse dimension within the death stranding reality which they simply need to die in to return to the "regular world".
This kind of makes sense of a few things for instance the WW2 soldiers/tank and the setting of the second trailer in the bombed out city, what if for instance this same anomaly that takes Reedus in 2018 say, is something not subject to normal temporal law but instead appears and disappears throughout time pulling objects in then disappearing. In other words: this black hole-esc anomaly appears at some point in the war and takes some of the dead 101st division soldiers, some tanks, some planes and part of an already destroyed city into the death stranding reality. It later appears and takes Norman Reedus, and later it appears at some point in the future and takes a bunch of technology from there. but the anomaly that seemingly jumps all around time in our universe is stable and constant in the death stranding universe. And as death has little meaning the formerly dead soldiers come back to life. As for why Mads is seemingly controlling them with psychic or cybernetic abilities I have no clue. But to me it makes a degree of sense, Norman Reedus is Stranded from our universe in a world were he cannot die and relics of all eras of humanity are pulled into.
Alternatively if you can't justify something like a black hole appearing that close to earth even for a picosecond and somehow not causing mass devastation I could argue it's Norman Reedus from a parallel universe to our own connected to the death stranding universe by some sort of time jumping black hole or just a made up anomaly.
So that's my two cents, anomaly connected to real world pulling people from different time periods into death stranding where humans can't die and everyone there is stranded from their own universe.
