Death stranding takes place on earth and/or in an alternative dimension to earth in a near future where humans have basically killed life on the planet.One of the main themes will be the battle between Life and Death.Babies that represent life are now like money.You pay for your life with them,GDT is infact meeting with Mads to "pay" him,he's not sure about that infact he esitates before connecting his umbilical cord to the pod.
Mads Mikelssen is not human,he can take every form he wants,he is one of those angelical beings we see in the first trailer or at least he has their same origins,these creatures can be defined as Aliens or Gods,they belong to another dimension and they feed themselves through life(that is a very rare thing on earth in DS) but they can also bring life to death,they represent the evil in the game,they are our enemies,because they bring life to have an advantage (the weird tank,planes with umbilical cords and the Skeleton army)and they bring war and fear to the world.Kojima confirmed that Women will be in the game and i guess that they will have a very important role.
Norman Reedus is also an "Alien" but in the opposite way than Mads ecc. He is in an alternate dimension that represents a Purgatory like world,this is the home of the Angelical beings,Kojima confirmed this through a tweet where he was listening to an album that belomgs to the movie "the man who fell on earth",infact this alternative dimension resembles earth.The baby represents a way to be connected to the real world for Reedus and he's maybe his son who was kidnapped/stole by the angelical beings.The handcuffs symbolize that too,but he has no belly button,this means that even if he was connected to the world,he's no more now,the angelical beings cut his rope,his connection to the world,to the baby.These of course are all my opinions and i'm sure that most of them will be false,but i enjoy doing this with a community that is very passionate about this game and that hopefully will like this theory
