Some previous unattempted concepts could be used:

Team Silent’s SH5 would have no fog, being a horror game set in the daylight (the original Texas Chainsaw Massacre was cited as an inspiration) and be about how “a day can rot away”

We can infer from the title that Silent Hills would’ve been about, well, multiple Silent Hills in some shape or form. My guess is that it also would have featured asynchronous online like Death Stranding in which you could affect other players games, again relating to the title

Guillermo del Toro also seemed to imply the game would have primarily take place in a house

If P.T. is anything to go by, the story would’ve involved MK Ultra somehow and might have had a first person perspective in some way, although I don’t think they’d get Norman Reedus’ likeness just to not show him off like they do in Death Stranding. Personally I liked the idea of Reedus as the protagonist though I certainly hope the final game wouldn’t have been filled to the brim with celebrity likeness like DS
I personally would like to see Team Silent’s original SH5 concepts be revisited
