In an opening flashback, Rick and Daryl are out on a supply run, a few days after defeating Negan and the Saviors. Daryl rides them both out on his bike and they stop at an old storage unit. While collecting what is inside, Daryl asks Rick how he is coping with Carl's death, to which Rick says that he takes it day by day. Rick admits the only thing keeping him going is Michonne and Judith and wants to honor everybody they lost along the way, but Daryl simply nods, keeping his opinions to himself. Daryl asks if Negan surviving is safe for his family and Rick believes it is. "All that matters to me is my family, Daryl. If I were to ever get separated from them, not even a whole army would be enough to stop me from getting back to them." Daryl hugs Rick, touched.

In the present, Jadis is flying off with the critically injured Rick with the CRM helicopter. After flying for hours, the helicopter arrives at it's destination in Philadelphia. The pilot lands the helicopter on a helipad at the CRM headquarters as Jadis is escorted off by a group of soldiers led by the military drill sergeant, Ivan Saddler. The CRM's head doctor, Wes Adamson brings the injured Rick to his hospital room to treat him while Jadis goes off to be interviewed. The helicopter pilot takes off his helmet and introduces himself to Jadis as Chase Gibson. Chase is furious that Rick was not bitten and reminds her that there are consequences for her deceit. She talks Chase into allowing her to speak with whoever is in charge. The major general of the CRM, Rodrick Beale invites Jadis into his office, where he interviews her for membership.

In Adamson's hospital room, his team desperately try to nurture Rick back to health, but reveals he is in a coma. Beale is willing to hear Jadis out, despite Chase's fury over her lies. Jadis tells Beale everything that happened in Virginia and demands she sees her friend, but he refuses to allow that until they are on the same page. Beale reveals what the CRM is all about and that they are all about saving people, but refuses to let her and Rick leave. He offers Jadis an offer to either join them and play a part within the organization, or be executed. Jadis agrees to comply. Adamson comes in to call Jadis in. Both of them alone in the room, Jadis whispers to Rick that she is going to break them both out of there when he wakes up.

Two years later, Rick wakes up, very similarly to how he did in Atlanta. Wandering around the building, Rick reunites with Jadis, who fills him in on what happened and that they are in Philadelphia. He immediately panics, running around and calls for Michonne and Judith, but Jadis says they cannot leave. Ready to go on a rampage, Beale enters and invites Rick into his office. After filling him in on what's happened, Rick demands he be brought back to his wife and daughter, but Beale refuses to let that happen and that he needs Rick, as he is a valuable asset to the CRM, as he is shown to survive the worst of injuries. When Rick tries to leave, Beale threatens to exterminate his family and friends in Alexandria, thus forcing Rick into submission. Beale has Rick signed up to join Sgt. Saddler's platoon for military training alongside a crew of new recruits.

Jadis gives Rick a tour of the headquarters. Rick is furious that Jadis would bring him there, but she reminds him that he would be dead if she didn't do what she did. Chase sees Jadis and the two kiss, revealing they have been in a relationship for the last six months. Chase's sister, Molly Gibson introduces herself to Rick and even acts somewhat flirtatious with him, but he walks off, thinking about Michonne. Rick searches all around for an escape route, but cannot find one. After finding a crawl space in one of the houses, he is ready to search inside, but he is stopped by Heath, who reveals Rick is searching in his house.

Heath and Rick catch up over a couple beers, with Rick updating him on everything that happened back home and that Denise, Glenn, Abraham, Sasha, Carl and many more died. He is saddened to hear this and apologizes for disappearing, but that he was traded in after being separated from Tara. Since then, he became a valued soldier trained by Saddler and promises to help Rick get by. Rick simply wants to get back home to his family, but Heath says once you're in, you're in.

In the barracks, Heath introduces Rick to the other soldiers. Before they can start talking, they are summoned by Saddler, where he meets the other men in training. Saddler breaks Rick into submission by making him do one hundred push-ups. A montage then ensues of Rick undergoing training for the military. Rick even accompanies Saddler on a mission to settle a dispute with a nearby settlement that wants Beale dead. He is forced to talk down the settlement's leader, Agatha Larson. After settling this dispute, Saddler is impressed with Rick's work and praises him up to Beale.

That night, Rick returns to the barracks, where the boys bond with one another, telling stories about their past. When it’s Rick’s turn, he simply says, “I’m looking for my family.” The soldiers start laughing at this, finding it somewhat corny, but one soldier in particular, Gregor Prince, relates to Rick’s predicament. Later on, Rick goes to bed and draws a picture of Michonne and Judith on an old iPhone that he found. Gregor invites Rick to go out for a smoke break and the two strike up a friendship. He reveals that he relates to him because he was separated from his wife and three kids. He often butts heads with Saddler, but vows to break out of the CRM one day and reunite with his family. With similar goals and ambitions, Rick and Gregor plan to work their way up into the CRM, assassinate Beale and return to their families.

Over the next few months, Rick and Gregor work their way up in the ranks and become a trusted soldier in Saddler's platoon. Impressed by his dedication, Saddler invites Rick into his office for a drink and asks him to help lead a mission in retrieving a runaway that escaped with supplies. Rick leads a group with Elizabeth Kublek and her group, to an island where the runaway named Amy is struggling to get back to her hometown. Amy captures Rick and strips him down, threatening to throw him overboard. She throws his belongings in the downstairs of the boat, (which is the one Michonne finds in Season 10). Rick is forced to prove his loyalty to the CRM by shooting Amy in the leg and brings her back to the CRM.

Beale expresses his admiration for Rick for the lengths he went to retrieve Amy. However, Rick doesn't feel like a hero and insists that what they are doing isn't right, but Beale blackmails him by reminding him about his family back in Virginia. When Rick leaves, he discovers Jadis is packing her things. He asks where she is going and she reveals that she was promoted as a warrant officer for the CRM and is going to lend her expertise elsewhere. Rick is confused as to why Jadis would bring him there just to leave him like that, but Jadis tells him when she joined Alexandria, she desperately tried to make amends for her wrongdoings, but people still distrusted her, aside from him and Gabriel. Now that she's within the CRM, she feels appreciated, more so than she was in Alexandria and is insistent on remaining with the CRM and working her way up. They bid farewell to one another.

That night, Rick lays in bed and has a flashback to two months after Season 8's finale. Rick visits Negan in his cell and tosses his sandwich to him. The two have a threatening conversation, where Negan says keeping him locked up for long enough can only fuel his anger and rage and gives him time to contemplate his revenge strategy. In the present, this resonates with Rick, who hallucinates Carl, encouraging him to fight so Judith doesn't have to grow up without a father. Rick says that if he leaves, everyone in Alexandria will die, but Carl says they don't have to if Rick can handle it. Inspired, Rick vows to break out of the CRM.

Rick meets with Chase, Molly and Gregor, who plan their escape out of the CRM headquarters. They also break Amy out of their prison, where Rick apologizes for shooting her and says he can help her reunite with her family. They recruit Agatha Larson and her settlement, delivering them weapons and explosives and hire them to launch an attack on the CRM headquarters that night. Agatha is reluctant at first, but ultimately agrees, as this is her chance to kill Beale.

That night, Rick's group puts their plan into motion: Rick sits down and talks with Beale, affirming his loyalty to the CRM and convinces Beale that he belongs with them. Beale is happy to hear this and they drink to that. Outside, Molly and Chase silently kill the guards near the entrances and open the gates to allow Agatha's group in to launch their recon mission. Gregor sets up explosives everywhere within the headquarters. After setting up the last one, he is held at gunpoint by Adamson, who demands to know what he's doing there. Gregor tells Adamson he doesn't want to hurt him, but Adamson is conflicted. After much hesitation, Adamson agrees to let Gregor go, but escapes the headquarters. Agatha's group is caught and a massive gunfight breaks out within the CRM headquarters, causing Gregor to detonate the explosives. As chaos ensues, Saddler kills Agatha's men and Beale is presumed dead in the chaos. Rick, Chase, Molly, Gregor and Amy escape the CRM headquarters in Chase's helicopter as they fly away towards Virginia. However, the helicopter was shot up badly in the process and are forced to abandon it and walk on foot. They celebrate their survival, but Rick gives a speech, telling them that the worst is still to come and they have a long way to go.

The film ends with a scene within the destroyed CRM headquarters. Agatha, having survived the fight, tries to reach Rick's group via walkie, but she is suddenly shot in the side and injured by Beale, who is bruised and bloodied. He viciously pins Agatha to the ground and reveals he knows she was conspiring against him with Rick. She orders him to tell him the job is done, to which she does. Beale then stabs her in the head and reunites with Saddler and his men as they stride away angrily on foot, rage-filled and vengeful.

In a post-credits scene, a group of people approach the destroyed CRM headquarters and are shocked to see what's happened to it. It's revealed to be Michonne and her group she left with. They wonder if there are any survivors as they advance towards the destroyed settlement.
