This isn't a detailed analysis or breakdown, just my idea of what the premise was for PT and going to possibly be for Silent Hills before it was cancelled.
In PT you play as Norman Reedus' character, and every time you move through the hallway you are traveling in a slightly different reality than your own: "Watch out, the gap in the door, it's a separate reality." The version of Norman Reedus you play is probably an overall decent, but troubled, character who might have (at most) become an alcoholic and abusive toward his wife when she had had an affair with her boss, and had his child. But the power of Silent Hill is transporting you and exposing you to alternate realities where your character does much worse things including murdering your wife and kids (the radio man talks about these murders). That murderer isn't you, it's an alternate reality version of you. ("The only me is me, are you sure the only you is you?")
I think in Silent Hills, the main antagonist was going to be an alternate reality version of your character, a much more evil and sinister version who ended up murdering his family. Maybe it would even manifest as a grotesque monster (kind of like Pyramid Head representing James' desire for punishment and guilt). I think your wife's ghost would haunt you as you enter different realities (like the ghosts of Walter's victims haunting you throughout SH4) and later on even the ghosts of your kids. The baby in the fridge and the fetus could represent the murder of your wife's illegitimate child. Basically, Silent Hills, like SH2, would delve into the consciousness of a troubled protagonist who is exposed to just how potentially evil he could have otherwise been and reveal to him his darkest thoughts in the form of horrible monstrosities. We won't ever know for sure, but it would have been a great experience regardless.
