I think this quote is talking about game mechanics.
Things like:
No hud
Having to balance the things you carry as you walk
Can actually get carried away by water, and can actually fall or trip
Worrying about boot degradation
Maybe Sam tires out after a while walking
A button to cover Sam's/Norman's mouth
Really immersive animations, dialogue while walking if you reach something cool or weird
At the start of the game, you AND Norman/Sam will have no idea what's going on, and you both learn the story at the same time (just a guess)
No death screens, which has been confirmed.......... No main menu?? Nor pause menu?? That would be pretty cool. No fourth wall breaks like that.
Some kind of indicator when enemies are nearby that relates to Sam's Chiral Allergy

Death Stranding; a secret Metal Gear Solid game? (Via YouTuber called TheTombPortable)-UNR
I know it seems like a giant leap to see Norman Reedus and a fetus and conclude it’s a MGS title but hear me out. Also, I’m just puking out what I saw from the YouTuber mentioned in my title, he deserves all credit for this.
The part of this very large video that very explicitly caught my eye is The Sorrow from Snake Eater. The Sorrow matches perfectly with a character shown in the Death Stranding trailer. This is kinda a big deal, but what caught me off guard was there is a scene where Naked Snake and Reedus’ actions are timed at exactly the same time. This happened after going through what looks like the exact same experience of near death, caused by the Sorrow. There’s way too much to this theory to write it out on reddit but I will attach the link to the original video.
